At direct IME we are committed to providing a superior brand of service.  Ensuring the integrity of all personal information we collect, use and disclose is an integral part of our service offering.  We support the industry-wide PiPEDA rules for the Protection of Privacy and the rules set out in Personal Health Information Protection Act, 2004.  We undertake to collect, use and disclose only that information required to meet our needs to perform our services.  We maintain excellent security systems and processes to safeguard the information that we collect, use and disclose.  All company employees are required to affirm their understanding of their responsibilities pursuant to direct IME's privacy policy.  Access to information is only granted to employees on an as needed basis.  Below is an outline of the procedures and policies that we follow in our company to ensure that our practices protect the integrity of personal information.  We do not sell or otherwise market personal information to third parties.  Further information regarding our Privacy Policy may be obtained by contacting direct IME’s Privacy Officer at 416-609-3211.

Privacy Statement

For the purpose of this Privacy Statement, personal information means:

information about an individual, including name, title, employment and personal contact information, personal information, occupational and social history information; information concerning physical, mental, vocational history and present status; health services information, information derived from testing or examination, information collected In providing our assessment services; information provided by the referral source, provided by the individual being examined or by a third party service provider; and all information that is collected.

Use of Personal Information:

direct IME only uses personal information in connection with performing its services; direct IME does not use personal information about an individual for any other purposes; direct IME may disclose a limited amount of personal information to third parties engaged to assist in carrying out our services as necessary. Third parties may include our rostered medical evaluators or transportation/translation vendors for example. These service providers are prohibited from using personal information for any purpose other than to provide requested assistance and they are required to protect personal information disclosed by direct IME in accordance with PiPEDA, and in accordance with all other applicable privacy laws. Personal information is also disclosed to the Referring Source who referred the individual for assessment to direct IME. direct IME does not disclose personal information for purposes unrelated to its services. direct IME reserves the right to disclose personal information to a third party if a law, regulation, search warrant, subpoena or court order legally requires or authorizes it to do so; direct IME also reserves the right to disclose and/or transfer personal information to a third party in the event of a proposed or actual sale or merger of business or assets of direct IME in order for that third party to continue to provide the services.

Knowledge and Consent

direct IME only collects personal information about an individual according to the law with regard to consent, and withdraw of consent, subject to applicable laws and reasonable notice; withdraw of consent may be facilitated by contracting direct IME's Privacy Office at 416-609-3211.
